There is an information form that needs to be filled out completely before the meeting. Download our form here — you need to print it out and fill it out in advance. If you choose to fill it out when you arrive, please arrive at least 15 minutes in advance.

The first 30 minutes of the meeting is complimentary, additional time is available at $147.50 per half hour. Payment is due immediately, and by cash, money order or check. We will not bill you. The meeting starts at the scheduled start time, not when you arrive.
